For any 3D related software (Quake) you need a Voodoo card (3dfx)

When you run qnxrtp.exe it will offer you the choice of create the floppy
for you. If you don’t create the floppy at installation time, you can
still
create it from the menu option in windows.

Same Problem here… 
The Terminal says ATH then ATM then some failure. I have an USR
Sportster Voice 33.6 Modem.
