Patch A "Unable to access /cd//boot/fs"

When I was trying to load Patch A from a booted CD I received the
following message: “Unable to access /cd//boot/fs Make sure the QNX
install CD is in your CDROM drive”. The CD boots fine and the first CD
of QRTP loaded fine. Could the double slash after the /cd be the
problem?
