What does this letter D mean during the boot process?
Is it bad (D)isk?
Is there possibly somewhere where all these codes
are documented?
I’ve seen ‘C’, I think? I think this is when
boot image goes over the 0124th (C)ylinder?
What are the others?

I don’t think so. I know of D and O (no AA55 signature in boot block)
from the primary loader and D and S (missing proc signature) from the
secondary loader.
