ChannelCreate(), _NTO_CHF_COID_DISCONNECT and _NTO_CHF_THRE

the release notes and the online documentation does not reports the modified
behaviour of these flags on QNX 6.3 from previous versions.

Apparently we changed the behaviour of the flags to match what the
documentation says they do, but the change didn’t get into the 6.3.0
release notes. I’ll update the notes.

In QNX versions pre 6.3 the docs is:

_NTO_CHF_COID_DISCONNECT

Setting this flag also results in the equivalent behavior of setting the
_NTO_CHF_THREAD_DEATH flag.
Only one channel per process can have this flag set.

_NTO_CHF_THREAD_DEATH

Setting the _NTO_CHF_COID_DISCONNECT flag also results in the equivalent
behavior of setting this flag.
Only one channel per process can have this flag set.

In QNX from version 6.3 this behaviour changed, the docs is ok i.e. these
notes are removed but the changed behaviour is not documented in the release
notes.
